When care needs become complicated, the difference between a good package and a poor one is rarely warmth. It is planning, training and communication. We build the package around the individual, train the carers for that specific person, and work alongside the professionals already involved rather than around them.

What Complex Care Means

There is no single definition. In practice, care becomes complex when needs are higher than routine support, when they change without warning, or when several professionals are involved and their instructions have to work together in one home.

It might follow a long term or progressive condition, a serious injury, a hospital admission or a gradual increase in dependency. What these situations share is that a standard package will not fit, and that getting it wrong has consequences.

What Complex Care Involves

  • A package built around one person

    Nothing standardised. The plan is written for this individual, their home, their routine and their risks.

  • Carers trained for the specific need

    Training is arranged for the package rather than assumed from experience. Specialist tasks are delivered only where staff are trained, competency assessed and signed off.

  • Personal care and daily living

    Washing, dressing, continence support, mobility and transfers, meals and hydration, with the same dignity as any other service.

  • Medication support

    Prompting or assisting with medication where assessed and permitted, with clear recording and prompt reporting of anything missed.

  • Working with the professionals involved

    Agreed responsibilities with GPs, district nurses and therapists, so instructions are followed consistently and nothing falls between services.

  • Escalation and reporting

    A written escalation route, accurate daily records, and prompt contact with the family and the right professional when things change.

How We Keep Complex Care Safe

The honest answer to "can you take this package" is sometimes no, and we would rather say so at the enquiry stage than accept work we cannot staff safely.

Before care starts we confirm what is needed, whether we can train and assess enough carers to cover the rota including sickness and leave, and who provides oversight for anything that requires it. Carers work unsupervised only once signed off. Where a task sits outside what our carers are trained and permitted to do, we say so and help identify who should be doing it.

How We Set Up a Complex Care Package

    Detailed enquiry

    We ask more questions than usual here, because feasibility matters more than speed.

    Assessment with everyone involved

    The person, the family, and where possible the discharge team, district nurses or case manager.

    Care plan and risk assessments

    Written for the individual, including the escalation route and who does what.

    Team building and training

    A small dedicated team, trained and assessed before the package starts.

    Early and ongoing review

    After the first week, then on an agreed schedule, and immediately whenever needs change.

Working With Professionals

We work with discharge teams, district nurses, case managers, continuing healthcare teams and commissioners across Cambridge and South Cambridgeshire.

We can review a referral, confirm whether the package can realistically be staffed and trained in the time available, and take part in multidisciplinary meetings. We will tell you early if a timescale is not achievable, rather than accepting a referral and letting a discharge fail.
